FOURTH GENERATION MOBILE PHONES (4G) INTRODUCTION: Since past some decades cell phone world has experienced a considerable amount of transition. This transition initiated from the First generation (1G) mobile phones carried forward by the 2G and 2.5G mobile phones. Nowadays, people are habituated of using the 3G technology. But the technologies and inventions are ever-restless and therefore the cell phone industry is again hopping with a great speed towards the Fourth generation (4G) technology. 4G mobile phone technology is a complete replacement of wireless communication in 3G technology. TYPES: There are 2 main types of 4G one of which is the Long-term evolution LTE and the other one is mobile WI-MAX. LTE: A natural extension of the current 3G technology is the Long-Term Evolution (LTE) which is also referred as the pre-4G technology. Transmission of all data including voice takes place as LTE is architectural design that can send all types of IP packets and data. â€Å"For a 20 MHz channel the downlink transmission bit rate of LTE up to 100 Mbps and 50 Mbps in the uplink and the bit rate capacity increases for a  Multiple-input multiple-output  (MIMO)†( 4G and Beyond 3G Technology,n.d. According to German (2010, March 15) some of the well-known mobile carriers in United States like VERIZON and ATT and several across the globe would convert their network to LTE and worlds first publicly available LTE-service was opened in the two Scandinavian capitals  Stockholm  and  Oslo  on the 14 December 2009. â€Å"VERIZON said that LTE testing in Seattle and Boston has gone well and that it will bring the technology to 25 to 30 markets this year† (German, 2010 March 15, Para 06).â€Å"According to the VERIZON, downloading speeds of 40Mbps to 50Mbps and upload speeds of 20Mbps to 25Mbps† (German, 2010 March 15, Para 06). However according to Nelson and Pica (2010, March 3) the average speeds will range from 5Mbps to 12Mbps for downloads and 2Mbps to 5Mbps for uploads. â€Å"In contrast ATT has announced that they will begin LTE testing in this commercial year and launch their network in the upcoming year 2011† (German 2010 March 15, Para 07). LTE is not only being planned to launch by the end of this year in United Stats but also the Telco Mobile One (M1) in Singapore conducted a demo of LTE technical trial. Also NTT Do Como will the first to introduce LTE in Japan this year itself. WI-MAX: WI-MAX is elaborated as Worldwide Interoperability for Microwave Access, and unlike LTE it is not an extension of current cellular system rather its more related to current Wi-Fi technology. â€Å"The initial version for mobile use is based on the 802.16e wireless standard (Wi-Fi is 802.110)† (German, 2010 March 15, Para 11). It has potential for very long range transmission (up to 30 miles) and could offer speeds of about 10Mbps† (German, 2010 March 15, Para 11). Where all the majority mobile carriers are interested in using the LTE the only US carrier to adopt the Wi-max technology is SPRINT. SPRINT has a Wi-max network in 27 cities and shows potential average speeds of 3Mbps to 6Mbps with maximum speed up to 10Mbps. But TDMA is less efficient as it is unable to handle high data rate channels. This is because TDMA requires large guard periods to improve the multipath impact. Similarly there were problems with FDMA as it used more bandwidth to avoid inter-carrier interferences. Thus to overcome these problems in the 2G technology one set along with the combination of TDMA and FDMA other set of access scheme was introduced which is known as the CDMA i.e. Carrier division multiple access. â€Å"Thereby the system capacity was increased but as a drawback placed a soft limit on it rather than the hard limit (i.e. a CDMA network will not reject new clients when it approaches its limits, resulting in a denial of service to all clients when the network overloads)† (Rumney M, 2008) . Data rate is increased as this access technique is able to manage multiple path channel. â€Å" This enabled the third generation systems, such as  IS-2000,  UMTS,  HSXPA,  1xEV-DO,  TD-CDMA  and TD-SCDMA, to use CDMA as the access scheme†(Rumney M, 2008). Although CDMA technique seems to be good so far yet it suffers from poor spectral flexibility and computationally intensive time domain equalization for wideband channels. More importance is given to the OFDMA i.e.Orthogonal FDMA, IFDMA i.e. Interleaved FDMA, SFDMA i.e. Single carrier FDMA and MFDMA i.e.Multicarrier FDMA because of the troubles in the traditional FDMA, TDMA and CDMA. The later used access schemes are based on efficient FFT (Fast fourier transform) algorithm and frequency domain equalization which would result in lower number of multiplications/s. Also bandwidth can be controlled and a spectrum can be formed in a flexible way. IPv6 SUPPORT: Circuit switched and packet switched networks are the base of the infrastructure of the 3G where as 4G will be based on packet switching only which would require low latency data transmission. Till the time when 4G will be introduced in the market and be a most used device the process of IPv4 address will be exhausted as a result a new version will be essential so that more wireless enabled devices can be supported. So IPv6 is be used to accomplish this task. â€Å"By increasing the number of  IP addresses, IPv6 removes the need for  Network Address Translation  (NAT), a method of sharing a limited number of addresses among a larger group of devices, although NAT will still be required to communicate with devices that are on existing  IPv4  networks† (Morr, D 2009). SOFTWARE-DESIGN RATIO (SDR): â€Å"SDR  is one form of open wireless architecture (OWA)† (Rumney, 2008). Progress or Alienation Our society has alienated itself far from the reality of the way things are and the way they should be, through the use and misuse of scientific knowledge and technology. Science is defined as, â€Å"a logical organized method of obtaining information through direct, systematic observation.† Sometimes science does not seem organized, in fact it seems like it opens us up to a different realm of possibilities that have consequences far beyond our wildest dreams. Scientific knowledge is something that sometimes cannot be controlled or monitored, but needs to be for the sake of the greater population. Those with the most power, for example political leaders and corporation giants, are often allowed privileged information that could jeopardize the safety of all of us. Now whether or not this information is taken in good faith, or for the almighty dollar doesn’t mean its right, nor does it mean that we should not explore scientific possibilities. Science stimulates our minds and forces us to use critical thinking and analysis based on our previous knowledge. Not all scientific information is wrong or incurs consequences, but like all data there is a right and a wrong way to distribute it. Scientific progress on the other hand is what has helped out society gain the knowledge and insight to live better lives through the advances in medical technology, the strategy of war, and the exploration of space. Not all scientific knowledge is misused, and it’s only brought to our attention when it has been. When this occurs people often question the validity of scientific work which leads to criticism. Some scientific progress will bring with it disruptive change in our society, but with change comes progress and the hope that we can better our lives. In the two stories I will present in this paper, Mary Shelley’s â€Å"Frankenstein† and Catherine Asaro’s â€Å"The Veiled Web,† they discuss the negative consequences of the actions from people who try and offer good insight to the scientific community and the general population. In both stories, two men take it upon themselves to manipulate science for the good of mankind. Both believe that good will come from their actions but neither consider the consequences of failure. The men in these stories are intent on their work and do not realize that others will turn it against them for destructive purposes. In â€Å"Frankensteinâ€Å", Victor Frankenstein realizes the destructiveness of his behavior, when it’s too late, and regrets it immensely.

Staff Development Teaching for Preventing Falls in the Elderly Long Term Care Facilities

In the United States, unintentional falls are the most common cause of nonfatal injuries for people older than 65 years. Up to 32 percent of community individuals over the age of 65 fall each year. Females fall more frequently than man in this age group, especially due to the fact that most female elderlies have bone weakness such as osteoporosis, which makes the fall to cause fatal injuries sometimes. Fall related injuries are the most common cause of accidental death in those over the age of 65. Approximately 40 fall related deaths per 100,000 people per year.Thus, fall are a growing public health problem that need to be addressed. Patient would not only suffer from such fall but the facility goes through hardships as well to deal with the patient. Fall related death rate in elderly group has been increasing in USA. Recently, falling has been occurring a lot in the long-term facilities. Falling has the highest rate of incident in the long-term facility because most of the patients are elderly. Falls can cause serious injuries and accidental death. In order to prevent falls in the elderly is very important.There are several different approaches to deal with predispose factors for elderly type of injury. I am going to reviews the major risk fators of falls and how to prevention for elderly populations living in long term care facilities. The effective falls intervention programs should take a multifaceted approach.Assess the staff for ability of learning and understandingI explained the background of important fall precaution to our licensed nurses and CNAs, the ones who gives care to the patients with ADL most closely. The Staff members, Licensed Nurses, and CNAs have different nationalities with different language. Licensed Nurses are 70% LVN and 30% RN. The experience in the healthcare facility for most of them is at least 3month. Because 90% of staff can fully understand in English, it is not necessary for the educational materials to be in other languages than English. The learners already are familiar with this topic because they already have had experiences in long-term facilities.How to educate and when is the best time for as much as possible participated in service. In order to prevent further falls, fall precaution presentations will be held during the weekdays in the conference room. Staffs can come in 30 minutes before or after the shift change, and spend an hour on this topic. First, I will show some fall cases through video, so I can induce the staff’s interest in this subject through visualization. Even though the staffs are well aware of the fall incidents, it is not easy to keep an eye on every patient 24/7, which can be frustrating and tiring to the staffs. However, I can explain how much this is important and tell them to try to understand and do the best. I will be identifying what risk factors or what kind of patient has high risks of falling using a current published brochure and video.What is the cost effect of a fall?Fall related injuries among elderly are associated with economic costs, which is much greater than the cost to implement a fall prevention program. In the long-term care facilities, even with an interdisciplinary team with physicians, nurses, social worker, physical therapy and administrators, fall prevention is still difficult because there is no way of knowing when one of the patients is going to fall. That is why it is crucial to educate staffs and patients to help reduce these barriers over time. Fall-related injuries account up to 15 percent of re-hospitalizations in the first month after the discharge from hospital. Falls carry staggering economic costs. Annual acute-care costs related to falls are estimated at $1.08 billion and long-term care costs at $4.9 billion.According to the Centers for Disease Control and Prevention, medical costs related to falls totaled more than $19 billion in 2007–$179 million for fatal falls and $19 billion for nonfatal fall-rela ted injuries. By 2020, the annual direct and indirect cost of fall injuries is expected to reach $54.9 billion. While falls have a tremendous impact on the patients, they also directly affect a healthcare organization’s cost per case and length of stay. Injuries from falls lead to a 61% increase in patient-care costs. Nearly every nurse can recall an incident in which a patient fell or nearly fell. As patients continue to age and present with increasing vulnerability and comorbidities, their potential  for harm increases.Predisposing factorsMulti-factorial Medications: Lots of medications such as benzodiazepine, tricyclic antidepressant, selective serotonin uptake inhibitors, and trazadone should be identified as high risk for fall. Most elderly long term care residents are reffered to as â€Å"polypharmacy† which is high risk for fall. Education (reorientation): fall prevention programs should be reoriented as much as possible call to increase safety awareness and r educes the fear of falling. Environment: frequently check environment that is very important. Web areas, clutter, poor lighting, and other environmental factors can reduce the risk of falling in nursing home.How to prevent fall1. After completed fall assessment upon admission, initiate fall precaution as soon as possible. Before appropriate technology and equipment can be chosen to help prevent falls, the patient’s fall risk, functional readiness, and mobility must be assessed. Most of patients consider for high risk for fall. Post-fall assessment is important because a lot of incidents are related to fracture, which is a serious matter. Fall prevention interventions should be assigned that are appropriate for individual based on the result of fall risk assessment2. Educate patients about predisposing and precipitating factors. This supports them to understand and enable them to do multi-disciplinary approaching. It should be built on initial risk screening results and involv e not just nurses but an interdisciplinary team of physicians, pharmacists, and physical and occupational therapists.3. Educate about basic knowledge of medication to CNA who is closely giving care than others.Essential implementation of safety caring in long term care facility1. Transfers: Patients rise from a sitting position to a partial stand to keep the center of gravity relatively low. Transfer the patient to the stronger side with the wheelchair at approximately 45-degree angle from the bed. If patients are unable to bear weight through the lower extremities due to weakness, place one end of the transfer board under the patient between the  buttocks and back of the thigh, then place the other end in the seat. Have the patient push up with the arms while slightly lifting the buttocks and slowly moving toward the wheelchair.2. Get belts: during transfer, obtain a standing position, turn and sit onto another surface or stand and walk to a new location, then turn and sit onto a new surface. The belt is better choice than the alternative placing one arm under the patient’s arm for the lift and holding onto the patient’s clothing or gown, which can injure the arm or shoulder, but care individual need to basic training from PT/OT personal.3. Bed and toileting safety: height- adjustable beds, safety rails, and raised toilet seats can reduce fall. To promote safety when the patients stands or for transfers, raise the bed. Otherwise, a weak patient who tries to stand could fall. Transfers onto and off a toilet, fix raised toilet seats and safety rails on either side of the toilet are needed. Patients can hold onto these to steady themselves when transitioning from a standing to a sitting position, and to push off from while standing after toileting. If the patient is unable to safely ambulate with assistance to the bathroom, provide a bedside commode.4. Mobility devices: mobility devices such as canes, walkers, and wheelchair mobility should be ma de sure that it has been evaluated and deemed approporiate and in a good working order. If it isn’t proper working contact a physical therapist for further evaluation. All mobility devices should be adjusted to the patient’s height and other characteristics as appropriate.5. Walkers: Nurse can walk next to the patient as the patient ambulates. Walker with seat must lock the brakes when ready to sit or transfer and complete the activity. Patients who become dizzy from nauseas or orthostatic can easily turn and sit on the seat without falling. This arrangement is much safer than having another staff member trail behind with a wheelchair and lower the patient to the wheelchair seat if patient becomes fatigued or dizzy or loses balance.6. Wheelchairs: individualized wheelchair prescriptions must be customized to each patient because the standard wheelchair does not work for all patients. For example, a dementia patient with memory deficits cannot be expected to remember to lock the wheelchair.What to do for caring for high risk fall patient1. Visual check every 2hours or more often 2. Keep bed in low position all the time 3. Lock wheels, call light, water pitcher, urinal in easy to reach. 4. Provide well lit path 5. Check shoes ; check well fitting, floppy shoes or loose clothing 6. Keep room clutter free, floor dry ad not slippery 7. Check bed, wheelchair alarm, or bedside mattress as ordered 8.
